KNOX COLLEGE STUDENT-ATHLETE REQUIRED FORMS
Required annual NCAA Forms are distributed through Front Rush. Student-athletes will receive directions via email when forms are available for each academic year. All student-athletes are required to complete the NCAA paperwork every year and are not permitted to practice or compete until forms are complete.
 
 Know what you sign:

• 2020-21 Student-Athlete Statement 
• 2020-21 Drug Testing Consent
• 2020-21 Student-Athlete Authorization/Consent for Disclosure of Protected Health Information for NCAA-Related Research Purposes
• 2020-21 General Amateurism and Eligibility Form for International Student-Athletes (Required for first-year/new international student-athletes)

 
All student-athletes must also be medically cleared to participate by the Knox College Athletic Training Staff. Visit the Athlete Physicals and Required Forms page for further details.
