Athlete Physicals and Required Forms

Preparticipation Athletic Physical Exam (all student-athletes)
Before you begin any practice and/or organized workouts, all athletes must have an annual physical exam completed by a qualified physician, physician assistant, or nurse practitioner. We are asking for each athlete to have that physical performed prior to coming to campus. If you are unable to have a physical exam performed prior to coming to campus, please contact your athletic trainer for assistance.

Please download the Knox College Athletic Physical Form (PDF) and bring it to your physician for completion. Once completed, submit your physical in your SportsWare Online Athlete Portal. Other Required Forms
  • All student-athletes must submit a copy of both sides to any applicable insurance cards in the Insurance section of their SportsWare Online Athlete Portal.
  • First time athletes must submit their Sickle Cell Trait status before participating in any intercollegiate athletics including strength and conditioning, practice, try-outs, or competitions.  
  • If you have a diagnosis of Attention Deficit Hyperactive Disorder (ADHD) or have received a diagnosis that requires the use of an NCAA banned medication, please fill out the ADHD Medical Exception Form or the NCAA Medical Exception Reporting Form.
  • If you will not be 18 years of age at the start of the athletic and/or academic year, please fill out the Minor Student-Athlete Consent for Medical Treatment form. This document will be used to provide day-to-day care of the student-athlete by the medical staff in the Athletic Training Room at Knox College and in emergency situations by other licensed medical providers when the parent/guardian is unable to be contacted.

All forms should be submitted in your SportsWare Online Athlete Portal by August 1st.

Yearly Athletic Medical Fee
Student-athletes will be billed $300.00 annually (regardless of the number of sports in which they participate) to help offset the costs of athletic medical services such as the athletic group insurance and other athletic medical services, such as the Team Physician and additional Athletic Training staff.  This charge will be billed to the student’s account and will be non-refundable.  The charge shall be applied after the first organized, supervised athletic activity (i.e., strength/conditioning training sessions, sport-specific practices, technique/individual work sessions, etc.) for the first sport participated in, whether in-season or out-of-season training.  The fee shall be applied to the student’s account regardless of whether the student-athletes participates in competition. The combination of the Athletic Insurance and Student Insurance plans ensures that every Knox student-athlete has adequate insurance coverage up to the NCAA catastrophic plan.
